Medical Screening & Intake Process
Your safety is the foundation of everything we do.
Ibogaine is a plant medicine with profound healing potential, but it is not suitable for everyone. Ibogaine has well-documented effects on the heart's electrical activity, and in some cases these effects can be serious. For this reason, we require a thorough and systematic medical screening before anyone participates in an ibogaine session. This is not a formality—it is a fundamental safeguard for every participant's life.
What the Screening Includes
Before you can be cleared to participate, you will need to complete the following medical evaluations:
1. 12-Lead Electrocardiogram (ECG/EKG)
Ibogaine and its metabolite noribogaine block the hERG potassium channel in cardiac muscle cells, which can prolong the QT interval. This can lead to ventricular tachycardia, Torsades de Pointes, and in rare cases, cardiac arrest. Research has shown that QTc prolongation beyond 500 ms can occur in approximately half of patients receiving ibogaine treatment. An ECG is therefore a mandatory baseline screening requirement.
We specifically evaluate:
- **QTc interval**: A QTc ≥450 ms in men and ≥460 ms in women is considered prolonged and represents a clear risk factor.
- **Presence of structural heart disease, bradycardia, or conduction abnormalities.**
- **Electrolyte status** (potassium, magnesium, calcium) and its effect on cardiac electrical activity.
If you have a known long QT syndrome, cardiomyopathy, history of myocardial infarction, arrhythmia, or cardiac hypertrophy, you are generally not a candidate for ibogaine treatment.
2. Comprehensive Metabolic Panel (CMP) Blood Test
Ibogaine is primarily metabolized by the liver enzyme CYP2D6 into noribogaine, and liver function directly affects how long the drug remains in your body and at what concentration. Impaired liver function slows metabolism, raises plasma ibogaine levels, and increases the risk of cardiac toxicity.
**A comprehensive metabolic panel includes:**
- **Liver function markers**: AST, ALT, ALP, GGT, bilirubin—to assess hepatic metabolic capacity.
- **Kidney function markers**: Creatinine, BUN—to assess renal clearance.
- **Electrolytes**: Potassium, magnesium, calcium—electrolyte imbalances can worsen QT prolongation and arrhythmia risk.
- **Blood glucose and other metabolic markers.**
3. Medication and Medical History Review
Ibogaine interacts dangerously with many medications. You will be asked to disclose all medications you are currently taking or have recently stopped, including:
- **QT-prolonging drugs**: SSRIs, SNRIs, tricyclic antidepressants, antipsychotics, methadone, certain antibiotics.
- **CYP2D6 inhibitors**: Certain antidepressants, antipsychotics, and beta-blockers—these can significantly raise ibogaine blood levels and increase toxicity risk.
- **Opioids**: The interaction between ibogaine and opioids can potentiate opioid toxicity, and several deaths have been associated with this combination.
- **Alcohol, benzodiazepines, and other substances.**
You will also be asked to disclose all past and current medical conditions, particularly cardiac history, liver disease, and psychiatric history (including psychotic symptoms, bipolar disorder, or seizure history).
4. Substance Use Assessment
If you are currently using opioids, alcohol, benzodiazepines, or other substances, we need to understand your pattern of use and your withdrawal risk. The interaction between ibogaine and opioids is particularly dangerous, and several fatalities have occurred in individuals with a history of opioid dependence.
5. Pregnancy and Breastfeeding
Pregnancy and breastfeeding are generally considered contraindications for ibogaine treatment, and this will be confirmed during screening.
Why These Tests Matter
Ibogaine's cardiac toxicity has been confirmed in multiple systematic reviews and case reports. A 2022 systematic review classified ibogaine adverse events into acute (within 24 hours) and persistent (beyond 24 hours), with cardiac events—most commonly QTc prolongation—being the most prominent concern. Some fatalities have occurred in **individuals with no known history of heart disease**, ranging in age from 19 to 64.
**This means that feeling healthy is not enough to determine safety.** An ECG and blood tests provide objective data, not subjective impressions.
How Screening Results Are Handled
After completing the above evaluations, our medical team will make an integrated assessment:
1. **Approved for participation**: All markers within safe ranges, no contraindicated medications or conditions.
2. **Conditionally approved**: Requires medication adjustment, electrolyte supplementation, enhanced monitoring, or written clearance from your primary care physician.
3. **Recommended to postpone or excluded**: Clear contraindications exist (e.g., significantly prolonged QTc, severe liver impairment, contraindicated medications that cannot be stopped), or the risks outweigh the potential benefits.
The Principle of Honesty in Screening
We do not judge your history, your medications, or your diagnoses. Our only goal is to ensure that ibogaine can do its work under safe conditions. **Concealing information does not protect you—it only places you at unnecessary risk.**
The Waiver Upon Arrival
Upon arrival at the treatment location, you will read and sign an **informed consent and liability waiver**. This document confirms that:
- You have disclosed all relevant medical information completely and truthfully.
- You understand the known risks of ibogaine, including cardiac risks.
- You accept personal responsibility for your participation.
- You release us from liability, within the bounds of the law, for consequences arising from information you failed to disclose.
Our Commitment to You
Your medical information is used only within our screening team and is kept strictly confidential. We make no moral judgments about your past or present circumstances. If you have any questions about your medications or your health status, **please contact us before registering**. A conversation beforehand is far better than a regret afterward.
Your honesty protects you. Our screening protects everyone.
